The most common reasons for tooth extraction include:

When to extract a wisdom tooth?
Wisdom teeth represent a special case, as they often have additional indications for extraction.
This is because they are the last teeth to erupt, usually in an area with very little space at the back of the mouth. As a result, they may fail to fully erupt, becoming impacted, partially impacted, or growing at odd angles.
An impacted wisdom tooth remains entirely within the bone.
A partially impacted tooth emerges only partially through the gum.
There are no absolute rules for determining whether an impacted or partially erupted wisdom tooth should be extracted, each case must be evaluated individually.

The main reasons for determining whether or not a wisdom tooth should be extracted are:
Discomfort or pain:
A very common cause of pain is pericoronitis, which is inflammation of the gum around a partially erupted wisdom tooth.
In cases where pericoronitis occurs for the first time, you may decide to wait and see, as it could be a one-time event.
However, if it happens again, extraction is usually recommended.Misaligned tooth causing soft tissue irritation:
For example, repeated cheek biting due to poor tooth positioning.Misaligned tooth affecting the second molars:

Treating a cavity in a wisdom tooth is often difficult due to its location at the back of the mouth, making access and cleaning challenging.
NB: crowding of the incisors is NOT an indication to extraction!! We often see patients who ask us to check their wisdom teeth because since they are erupted other teeth have shifted. This is indeed a very common belief but has no scientific basis.
What is oral surgery still about?
Other interventions that are carried out less frequently are represented by :
Apicoectomy (endodontic surgery):
Removal of the tip (apex) of a previously root-canal-treated tooth if it shows signs of infection or pain.Sinus lift:
A procedure used to increase bone volume in the posterior upper jaw to allow proper implant placement.Osteoplasty:
Involves removing bone to create a suitable surface for implant placement.Periodontal surgery:
Includes increasing or reducing bone levels around a tooth for gum health and stability.Aesthetic procedures:
Such as removing or repositioning the gums to create a more attractive smile.
Does it hurt to undergo oral surgery?
This is a very common question, and the answer is: NO.

During the procedure:
A strong local anesthetic is administered to ensure the patient feels no pain.
In many cases, pre-surgical medication is also prescribed to reduce inflammation and enhance the effectiveness of the anesthesia.After the procedure:
Depending on how invasive the surgery was, medications and post-operative instructions are provided to keep discomfort under control.
As a result, most patients experience only mild discomfort.
A small amount of swelling may occur, but it typically disappears within a few days.
